What is Enhanced Care Management (ECM)?

ECM is an intensive care coordination benefit provided through Medi-Cal managed care plans. It helps members with complex health or life needs obtain appropriate care and community support at the right time and place.

Does it cost money to apply for ECM?

Eligible Medi-Cal managed care members do not pay an additional fee for ECM care management services.

What if I do not have a Medi-Cal health plan?

You must first enroll in a Medi-Cal managed care plan. For assistance, call Medi-Cal Health Care Options at 800-430-4263.

How do I know if I can apply for ECM?

You must be enrolled in a Medi-Cal managed care plan and meet the criteria for a designated Population of Focus. Ask your doctor or health plan, or call SunnyDay ECM at 626-350-3886.

Will I lose my Medi-Cal benefits if I join ECM?

No. ECM adds care coordination and support and does not replace your current Medi-Cal medical services.

What does ECM offer, and how can it help me?

A dedicated care manager helps create a care plan, arrange and coordinate medical services, support care transitions, and connect you with appropriate community and social resources.

Do I need to change my primary care doctor?

No. You may continue seeing your regular doctor. The ECM team helps coordinate communication with your doctor and other care providers.

Can I stop ECM after I join?

Yes. ECM is voluntary, and you may contact us at any time for help stopping services.

Can I change my ECM provider or lead care manager?
