Additional care coordination and support for eligible Medi-Cal managed care members.
Enhanced Care Management (ECM) is a Medi-Cal managed care benefit. Eligible members who voluntarily enroll receive help from a dedicated care manager and care team to coordinate medical care, behavioral health, social services, and community resources.
Joining ECM does not replace your current Medi-Cal benefits. You may continue seeing your regular doctors, and you may choose to stop ECM services at any time.
Who May Be Eligible?
You must be enrolled in a Medi-Cal managed care plan and meet the plan’s criteria for a designated Population of Focus. This may include people with complex medical or life needs, frequent emergency room or hospital use, care transitions, or risk of institutional or long-term care placement. Final eligibility and services are determined and authorized by your health plan.

Four Key Areas of SunnyDay ECM Support

Meal Support
Subject to member eligibility, health plan authorization, and program requirements, we help connect members with medically supportive meals or other appropriate nutrition resources.

Medical Coordination
We help arrange appointments, follow-up care, interpretation, and communication so members can more easily obtain needed medical services.

Housing Resources
Based on need, we help connect members with senior housing, application information, and eligible housing-related community supports.

Transportation and Benefits
We help coordinate appropriate transportation for medical appointments and provide information and coordination for IHSS and other social or community resources.
Frequently Asked Questions
Does it cost money to apply for ECM?
Eligible Medi-Cal managed care members do not pay an additional fee for ECM care management services.
Will I lose my current Medi-Cal benefits if I join?
No. ECM provides additional coordination and support and does not replace your current Medi-Cal services. You may continue seeing your regular doctors.
Can I stop ECM services?
Yes. ECM is voluntary. You may contact us at any time for help opting out or changing providers.
