SunnyDay Enhanced Care Management
Additional care coordination and support for eligible Medi-Cal managed care members.
Enhanced Care Management (ECM) is a Medi-Cal managed care benefit. Eligible members who voluntarily enroll receive help from a dedicated care manager and care team to coordinate medical care, behavioral health, social services, and community resources.
Joining ECM does not replace your current Medi-Cal benefits. You may continue seeing your regular doctors, and you may choose to stop ECM services at any time.
Who May Be Eligible?
You must be enrolled in a Medi-Cal managed care plan and meet the plan’s criteria for a designated Population of Focus. This may include people with complex medical or life needs, frequent emergency room or hospital use, care transitions, or risk of institutional or long-term care placement. Final eligibility and services are determined and authorized by your health plan.
