SunnyDay Adult Day Health Care, located in El Monte in Los Angeles County, is dedicated to providing professional, comprehensive, person-centered daytime health care for older adults and other adults with care needs.

Our interdisciplinary team includes nurses, social workers, psychological counselors, rehabilitation professionals, dietitians, speech therapists, and activity staff. Based on each participant’s health condition, functional abilities, and individual needs, the team develops an appropriate care plan. Professional services include:

Professional Nursing and Health Monitoring

Nursing staff provide health assessments, blood pressure and blood glucose monitoring, medication management, chronic disease care, health education, and coordination with physicians and family members.

Social Work and Emotional Support

Services include individual assessment, emotional support, family consultation, referrals to public benefits, and connections to community services, helping older adults maintain emotional well-being and social participation.

Rehabilitation and Functional Maintenance

Based on individual needs, professionals provide physical therapy, functional activities, exercise training, and fall-prevention support to help participants maintain mobility, independence, and the ability to perform daily activities.

Varied Activities and Social Participation

Fitness, music, dance, arts and crafts, cognitive games, holiday celebrations, and health lectures enrich participants’ lives, promote physical and emotional wellness, and reduce loneliness and social isolation.

Personal Care and Safety Assistance

According to each participant’s needs, staff assist with activities of daily living, personal hygiene, mobility, and toileting, providing appropriate care in a safe, respectful, and dignified environment.

Balanced meals, snacks, and nutrition education are provided according to each participant’s health status and special dietary needs, supporting the management of diabetes, high blood pressure, and other chronic conditions.

Transportation Services

Safe transportation between home and the center is available to eligible participants, making it convenient and reassuring to attend daily activities.

CalAIM Enhanced Care Management and Community Supports

Through SunnyDay Enhanced Care Management and Community Supports, eligible members receive help coordinating medical care, behavioral health, social services, and community resources—reducing barriers to care and improving quality of life.
